Position Summary

The Community Health Supervisor provides leadership, supervision, and operational oversight for Community Health programs,  The position is responsible for managing Community Health Workers and related staff, overseeing community care coordination and referral workflows, digital navigation, health literacy, and ensuring compliance with project contracts, and fostering strong community partnerships.

The Community Health Supervisor leads efforts to address social determinants of health by connecting patients to essential community resources, coordinating care across healthcare and social service systems, and supporting whole-person care. This position collaborates closely with clinical teams, community organizations, managed care organizations, and regional partners to improve health outcomes and reduce barriers to care for vulnerable populations.

Minimum Qualifications: Bachelor's degree in Social Work, Public Health, Human Services, Healthcare Administration, or related field preferred; equivalent experience may be considered. Minimum three years of experience in community health, care coordination, social services, population health, outreach, or a related field. Knowledge of social determinants of health, care coordination models, Medicaid programs, and community resource networks.  Strong project management, organizational, and problem-solving skills. Excellent written and verbal communication skills. Proficiency with electronic health records, data collection systems, and Microsoft Office products. Must possess a valid driver's license and acceptable driving record.  Candidate has not been sanctioned or excluded from participation in federal or state healthcare programs by a federal or state law enforcement, regulatory, or licensing agency.

Desired Qualifications: Previous supervisory or leadership experience. Knowledge of community resources and service systems throughout Yakima County and the surrounding region. Experience working with underserved populations, including individuals experiencing homelessness, agricultural workers, immigrants, refugees, and low-income populations. Experience with grant-funded programs, contract management, performance monitoring, and reporting requirements.  Bilingual Spanish/English.

Essential Duties and Responsibilities

Leadership and Staff Supervision

  • Provide day-to-day supervision, leadership, coaching, mentoring, and performance management for Community Health Workers and assigned Community Health staff.
  • Recruit, orient, train, and support staff development and professional growth.
  • Conduct regular staff meetings, individual supervision sessions, and performance evaluations.
  • Establish accountability measures and productivity expectations.
  • Ensure compliance with organizational policies, procedures, confidentiality requirements, and regulatory standards.
  • Foster a culture of teamwork, inclusion, innovation, accountability, and patient-centered service.

Community Coordination HUB Oversight

  • Provide operational oversight of patient connection to social needs such as housing, education, employment, legal help, child care, food, financial help, and other basic needs designed to improve self-sufficiency. Oversee completion of patient referrals and follow through of service delivery and documentation of results.
  • Ensure compliance with policies, procedures, contract requirements, quality standards, and reporting expectations.
  • Monitor referrals from assignment through completion to ensure timely outreach, engagement, documentation, and closure.
  • Work collaboratively with contractors and  community partners to strengthen referral systems and improve community care coordination outcomes.
  • Monitor staff workload, referral volumes, and program performance to ensure effective service delivery.
  • Ensure accurate and timely documentation of referrals, encounters, and outcomes in required systems.
  • Identify and resolve barriers that affect successful referral completion and client engagement.

Care Coordination and Social Determinants of Health

  • Lead community-based interventions addressing social determinants of health, including housing instability, food insecurity, transportation barriers, employment needs, education, financial challenges, legal concerns, and access to healthcare.
  • Assist staff in developing comprehensive plans to support clients and families.
  • Coordinate with healthcare providers, behavioral health providers, social service agencies, schools, housing programs, and community-based organizations.
  • Support care teams in identifying and addressing barriers that negatively impact health outcomes.
  • Advocate for patients and families to ensure equitable access to community resources and services.
  • Promote patient self-sufficiency through education, coaching, and resource navigation.

Program Operations and Contract Management

  • Oversee implementation of grants, contracts, work plans, and special initiatives related to community health and community coordination.
  • Monitor program deliverables, outcomes, timelines, and performance measures.
  • Track program performance and ensure compliance with funding requirements.
  • Prepare reports, summaries, dashboards, and presentations for organizational leadership, funders, and community stakeholders.
  • Assist in identifying funding opportunities and support grant development activities as requested.
  • Maintain policies, procedures, and workflows that support efficient and effective program operations.

Quality Improvement and Data Management

  • Monitor program effectiveness through outcome measurement and performance reporting.
  • Review data related to referrals, service utilization, care coordination activities, and community resource access.
  • Participate in quality improvement initiatives designed to improve patient outcomes and operational efficiency.
  • Develop and implement corrective action plans when performance goals are not achieved.
  • Ensure data integrity, timely documentation, and accurate reporting.
  • Use program data to identify trends, improve service delivery, and inform decision-making.

Training and Education

  • Provide training and technical assistance to staff regarding community resources, care coordination strategies, and community referral systems.
  • Support health literacy initiatives and promote patient engagement in healthcare services, patient portals, telehealth, and preventive care.
  • Ensure staff remain informed regarding healthcare coverage programs, eligibility requirements, and community service resources.
  • Participate in organizational training and professional development activities.

Additional Responsibilities
