We are looking for a team member that is passionate about serving our community of patients and enjoys working in a team! Some technical requirements for this position are:

  • Master’s or Bachelor’s Degree in a social service or advocacy related field such as: social work, behavioral sciences, political science, psychology, and at least three years’ experience in social services, public health services, and/ or client advocacy/ linkage program or another related field.
  • Convenient if bilingual in English/Spanish.
  • Candidate has not been sanctioned or excluded from participation in federal or state healthcare programs by a federal or state law enforcement, regulatory, or licensing agency.
  • Must have valid driver's license and car insurance.

Day to Day:

  • Train, supervise, and oversee Care Coordinators in all YNHS sites in principles of care coordination.
  • Monitor care coordinators activities (coordination, transitions, referral management, health promotion, individual and caregiver support) according to program regulations, documentation in YNHS EHR as well as external registries and forms.
  • Facilitation interdisciplinary case conference with the health care team as needed to maximize care coordination.
  • Match care coordinators to primary care teams to maximize efficiency.
  • Work closely with MA Supervisor(s), Nursing Director, and CMO to coordinate workflow and patient access to health services.
  • Maintain strict confidentiality standards in regard to mental health services and chemical dependency counseling standard.
  • Obtain appropriate and specific releases from clients before sharing information with other agencies.
  • Participate in quality assurance activities and peer review as requested.
  • Maintain organized, clean and confidential work area
  • Represent YNHS in local meetings as requested.
  • Perform related work or other tasks as requested.
