Our Ideal Candidate: We are looking for a candidate that has a passion to work with individuals who are homeless or in transitional housing The technical qualifications are:

  • Associate degree in behavioral or health sciences or related field.
  • Bilingual in English/Spanish.
  • Two years of closely related work experience.
  • Knowledge of effective case management practices.
  • Knowledge of government benefits and community resources.
  • Experience in effective social service delivery.
  • Knowledge of addiction and mental health pathologies.
  • Ability to successfully pass a background check in relation to the position applied for.
  • Must have valid driver's license and car insurance.

Day to Day:

     •   The Youth Services Case Manager implements and supports programs to support youth, and young adults, including YYA                who are homeless and at risk of homelessness.

     •   This position collaborates with an interdisciplinary team to offer holistic services in both individual and group settings,                supporting health, vocational, and self-sufficiency goals; works with community partners to access stable housing and                 other basic needs assistance, promotes healthy behaviors and positive youth development.

    •     Develop service plans with clients, advocates for youth voice, and works as a member of the Homeless Resource Team to            support community members securing housing and support services.

    •     Maintaining confidentiality of client interactions and medical records.

    •     Participate in staff in-service meetings.

    •     Participate in quality assurance activities and peer review as requested.

    •    Maintain organized, clean, efficient and confidential work area.

    •    Represent YNHS in local meetings as requested.
