Community Health Worker

We are looking for:

We are looking for a team member who is passionate about serving our community of patients and enjoys working in a team! The community health worker: 

  • Assists clients in obtaining health services and addresses barriers that may prevent access to care. This includes coaching in the use of technology to access patient portals and telehealth for medical, behavioral health, and social service care.
  • Assists clients in obtaining smartphones or other devices available through state/federal programs, including Managed Care Organizations, so they can utilize telehealth for health and social services.
  • Assists clients in scheduling virtual visits for medical or behavioral health, setting up their smart devices for virtual visits, and coaching them on how to conduct virtual visits independently with confidence. This may involve the CHW accompanying the patient/client at the beginning of several visits to successfully start each visit in a confidential area, then leaving the patient in a confidential space for the visit, and remaining available for technical help if needed.
  • Assists clients in accessing health care information via patient portals to obtain their health information, request medication refills, and communicate with their health care team using technology from their home or dwelling unit.
  • Provides assessments and screenings for social determinants using approved tools.
  • Assists clients in addressing other barriers to care e.g., language, transportation, insurance needs, etc.
  • Assists clients in addressing emergent needs e.g., food, shelter, clothing, etc.
  • Provides health education, as directed, using appropriate health aids.
  • Attends case conferences or morning huddles as needed to address the comprehensive needs of the client.
  • Contacts clients in the clinic, client dwellings, streets, or other outdoor settings.
  • Documents contacts and interactions in electronic health records.
  • Participates in quality assurance activities and peer review as requested.
  • Maintains an organized, clean, efficient, and confidential work area.
  • Represents YNHS in local meetings as requested.
  • Performs related work and other tasks as requested. 

Position details:

  • $20-23/hour
  • Full-time
  • This job requires that you are fully vaccinated against COVID-19
  • Candidate must have:
    • Knowledge of community services, social services, and community resources (required)
    • The ability to speak clearly and concisely
    • Be bilingual in English/Spanish; Proficiency in ASL is convenient
    • Candidate must not have been sanctioned or excluded from participation in federal or state health care programs by a federal or state law enforcement, regulatory, or licensing agency
    • The ability to successfully pass a background check for the position
    • A valid driver's license and car insurance
