Outreach Worker

About this opportunity:

We are looking for a team member who is passionate about serving our community of patients and enjoys working in a team! Our outreach workers are responsible for:

  • Identification and outreach to homeless people to determine who needs services
  • Needs assessments to help determine client limitations and strengths
  • Providing information about available services from YNHS and other agencies
  • Assisting with securing food, shelter, clothing, and referrals to other supportive services
  • Assisting in accessing entitlements and completing forms and applications
  • Developing individual care plans in consensus with staff and clients
  • Participating in crisis intervention activities as required
  • Coaching and supporting life skills and self-sufficiency skills
  • Advocating on behalf of clients
  • Documenting activities completely in electronic health record systems and other record-keeping systems as required
  • Maintaining confidentiality of client interactions and medical records.
  • Participating in staff in-service meetings
  • Participating in quality assurance activities and peer review as requested
  • Maintaining an organized, clean, efficient, and confidential work area
  • Representing YNHS in local meetings as requested
  • Performing related work and other tasks as requested

Position details:

  • Starting wage of $21-$24.15
  • Associate degree in behavioral or health sciences or a related field and two years of closely related work experience
  • Qualifying work experience may be substituted year for year for education 
  • Good attention to detail; good written and verbal communication skills
  • Bilingual Spanish/English desirable
  • Candidate has not been sanctioned or excluded from participation in federal or state healthcare programs by a federal or state law enforcement, regulatory, or licensing agency
  • This job requires that you be fully vaccinated against COVID-19
